About The Position

The Clinical Transition Specialist is an experienced sales professional who sells Option Care products and services to patients being discharged from care. The role educates patients, families, and facility staff about receiving services and products at an alternative site, and assesses patients to determine whether they are candidates for that care setting. The specialist gathers preadmission information and works with internal intake teams and external partners, including case managers and discharge planners, to support quality service and effective treatment plans. The position builds relationships with referral sources and partners with Account Managers to grow referrals and achieve sales goals. It provides in-person patient and caregiver education to support timely discharge and therapy independence, coordinates care transitions to home or an alternative site, and communicates with providers and other care-team members throughout the process. The role also reviews medical records, supports patient tracking and therapy starts, and helps arrange special medical supplies or appliances. The position requires travel to partner facilities, community hospitals, medical offices, and other care locations.
