The Clinical Transition Educator is responsible for providing clinical education and training to ensure successful transition of patients to an Option Care Health care delivery model. The Clinical Transition Educator will also be responsible for partnering with the sales team to increase the number of patients being transitioned to OCH care delivery. This role involves providing education and training to patients, caregivers, and facility staff about OCH services and products in alternative sites, at home, or virtually. It also includes conducting patient assessments, partnering with the sales team to identify educational needs and improve patient transition volume, maintaining confidentiality, documenting education, and coordinating patient transitions from hospital to home or alternate care settings. The role also involves providing in-person patient and caregiver education, gathering pre-admission and insurance information, and facilitating the initiation of services.
