The Clinical Transition Educator is responsible for providing clinical education and training to ensure successful transition of patients to an Option Care Health care delivery model. The Clinical Transition Educator will also be responsible for partnering with the sales team to increase the number of patients being transitioned to OCH care delivery. This role involves evaluating, educating, and training patients, caregivers, and facility staff about how OCH services & products will be facilitated in an alternative site, in-home, or virtually. The educator will conduct patient assessments, partner with the sales team to identify educational needs and improvements, and maintain confidentiality of patient information. They will also coordinate patient transitions from hospital to home or alternate care settings, provide in-person patient and caregiver education, gather pre-admission and insurance information, and facilitate the initiation of services for safe and effective home care.
