About The Position

Health Solutions is a premier wellness center focused on whole person care. We are hiring two Clinical Care Specialist, IYS Intensive Youth Services, located at our Family Center at 1591 Taos Road in Pueblo. What You’ll Be Doing As a Clinical Care Specialist at the Family Center you would be a vital contributor to working with children and youth in an intensive outpatient setting to promote healthy youth and families. Your work in case management, crisis intervention, and intensive outpatient group therapy to children, youth and their families means you would have rewarding opportunities to directly enhance the wellbeing of our community now and in the future.

Requirements

  • Baccalaureate degree in social work, psychology, education, or closely related field required.
  • Experience in conducting clinical interviews and triage assessments, family case management, and community systems navigation and experience working with children/families as well as school systems preferred.
  • Valid Colorado driver's license and access to transportation during working hours
  • Demonstrated computer proficiency

  • Take ownership of problems, brainstorm problem resolutions, and use sound judgment in selecting solutions to problems, and demonstrate consistent follow through.

Responsibilities

  • Facilitate and or co-facilitates quality intensive outpatient programming among clients ranging in ages from 4 to 16
  • Facilitate and or co-facilitates group therapy based on assessed need
  • Conducts case management in collaboration with resources in the community
  • Provides crisis intervention services during co-facilitated groups
  • Enhancing problem-solving skills
  • Linking the child/adolescent/family with community resources
  • Linking the child/adolescent/family with the schools
  • Providing/arranging transportation for scheduled appointments
  • Providing support to remain in home and community to avoid more restrictive, disruptive out of home placements
  • Maintains accurate and up-to-date records with 100 percent compliance with Health Solutions' standards
  • Maintains 100 percent productivity for billable services
  • Maintaining accurate, complete, and up-to-date records with 100% compliance with Health Solutions standards
  • Maintaining competency and proficiency with the electronic health record
  • Performing other job-related duties assigned by the director or his/her designee
  • Frequently remains active with clients for prolonged periods
  • Frequently moves within and between facilities
  • Occasionally moves equipment and/or materials up to 20 pounds
  • Frequently exposed to stressful or emergent situations
  • Constantly communicates with clients and other staff members
  • Occasionally exposed to communicable diseases or bodily fluids
  • Frequently uses computer and other office equipment to enter, manage or look up data
