About The Position

The BH Clinical Care Specialist plays a crucial role in providing comprehensive behavioral health support to patients, ensuring they receive the highest quality of care. This position involves assessing patient needs, developing individualized care plans, and coordinating with multidisciplinary teams to facilitate effective treatment. The specialist will also monitor patient progress and make necessary adjustments to care plans based on ongoing evaluations. Requirements

  • Bachelor's degree in psychology, social work, nursing, or a related field.
  • Valid state licensure or certification in a relevant behavioral health discipline.
  • At least two years of experience in a clinical or healthcare setting, preferably in behavioral health.
  • Relevant experience may substitute for the educational requirement on a year-for-year basis.

Nice To Haves

  • Master's degree in a relevant field.
  • Experience with electronic health record (EHR) systems.
  • Familiarity with evidence-based practices in behavioral health treatment.

Responsibilities

  • Conduct thorough assessments of patients' behavioral health needs and develop tailored care plans.
  • Collaborate with healthcare professionals, including psychiatrists, psychologists, and social workers, to ensure comprehensive patient care.
  • Monitor and document patient progress, adjusting care plans as necessary to meet evolving needs.
  • Provide education and support to patients and their families regarding treatment options and coping strategies.
  • Participate in case management meetings and contribute to the development of best practices in behavioral health care.
