About The Position

Health Solutions is a premier wellness center focused on whole person care. With over 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community. We’re hiring for a full-time Clinical Care Specialist to join our Adult Services department at 41 Montebello. As a Clinical Care Specialist, you will play a central role in helping our clients’ access services and take the next steps to wellness. Your work will take you to homes, and other settings, and call upon you to collaborate with other professionals within and outside our agency to assure that clients receive the very best care available.

Requirements

  • Bachelor's degree in social work, psychology, or closely related field
  • Computer proficiency
  • Valid Colorado driver license, good driving record and access to transportation during working hours
  • Must be able to remain in a stationary position at least 50% of the time
  • Frequently communicates with physicians about medical notes, must be able to exchange accurate information in these situations
  • Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer
  • Occasionally lifts and carries items up to 20 pounds
  • Occasional exposure to communicable diseases and bodily fluids
  • Must already be authorized to work in the US; sponsorships not available

  • Experience providing case management services to adults
  • Experience with community systems navigation
  • CAC training
  • Experience working with those experiencing co-occurring disorders
  • Spanish language skills

Responsibilities

  • Maintains a caseload and conducts case management services for clinicians
  • Makes appropriate referrals to address mental health and substance use needs
  • Links clients and community resources effectively
  • Offers behavioral health counseling and outreach
  • Ensures clients attend scheduled appointments in the community and at Health Solutions
  • Identifies stressors and precursors that led to the current situation or crises
  • Identifies available coping mechanisms and natural supports
  • Enhances problem-solving, independent living, and socialization skills among clients
  • Provides support for clients to remain in home and community to avoid more restrictive living arrangements
  • Maintains accurate records in the agency’s electronic health record
  • Performs other job-related duties assigned by the director or designee
