About The Position
The Clinical Care Partner MA is responsible for coordinating pre-visit patient care following established standards and practices of population health management. This role monitors the disease registry for patient-scheduled appointments, procedures, tests, and referrals, while communicating high-risk patient needs to a registered nurse care coordinator. The position relies on guidelines and experience to accomplish tasks and operates under close supervision.
