About The Position

Homebridge serves a high risk, high needs, complex-diagnosed population with a combination of domestic, personal, and paramedical homecare functions funded through IHSS. Our work allows an often-underserved population to live safely in their community. Homebridge offers a dynamic, fast-paced work environment with 350+ unionized field workers and 90+ internal office staff. Under the direction of the Director of Care Management, the Clinical Care Manager leads the clinical team in the provision of skilled medical training with staff, and with supporting client’s care transitions from Hospital or SNF to home. The Clinical Services Manager meets with Care Supervisors, Home Care Professionals (HCPs), clients, and community partners to provide feedback on client’s skilled care needs and provide training to staff to ensure care remains consistent. The Clinical Services Manager plays a critical role in helping older adults and people with disabilities to remain safe while living at home.

Requirements

  • Current and active LVN or RN license to practice in California required
  • Minimum of 3–5 years of experience required
  • Experience in a community social service or home health agency required
  • Experience performing: Case assessments, Client interventions, Care plan development
  • Proficient in Microsoft Windows, common office productivity tools including Microsoft Office 365 Suite (Outlook, Word, Excel, SharePoint, Teams) and Apple mobile devices (phones and tablets)
  • Proficient in navigating Electronic Health Records (EHR)
  • Knowledge in understanding the needs of clients with complex medical, mental health, and substance use disorder diagnoses
  • Must be a self-starter, able to work under pressure, prioritize, problem-solve, and meet deadlines
  • Ability to critically analyze, address, and solve complex problems
  • Ability to interpret a variety of instructions furnished in written, oral, diagrammatic, or schedule form
  • Is committed to the agency’s mission to deliver high-quality services
  • Maintains an active LVN/ RN license to practice in California
  • Ability to perform the full range of nursing duties and responsibilities in a patient care setting
  • Ability to write, interpret, and implement care plan activities

Nice To Haves

  • 5 years of equivalent work experience may be substituted for direct experience
  • Previous experience working with older adults and disabled communities preferred
  • Knowledge of older adult and disabled populations preferred
  • Experience training, coaching, or supervising caregivers or clinical staff preferred

Responsibilities

  • Develops and delivers hands-on training for HCPs on specific skilled paramedical client needs (e.g., ostomy care, insulin injections).
  • Skills Competency Validation: Formally test and sign off on caregiver skills to ensure compliance with clinical standards
  • Provides refresher or remedial training for staff who are unable to demonstrate skill competence.
  • Works closely with Care Supervisors and Lead Care Managers to coordinate client’s transfer from the hospital or Skilled Nursing Facility (SNF) to home.
  • Assesses clients in the hospital or SNF for change in care needs and identifies areas of support that the client may need support with post discharge.
  • Ensures that HCPs are receive training on skills to best support the client’s wellbeing once they are discharged home.
  • Reviews medication lists and discusses any medication changes or contraindications identified with the client, HCPs, and other members of the care team.
  • Completes documentation in the client chart regarding client needs post discharge from facilities.
  • Reviews and signs off on care plans for clients participating in Enhanced Care Management (ECM).
  • Provides clinical feedback and support to ECM case managers to ensure medical needs are safely met.
