Under indirect supervision, this role coordinates and provides comprehensive patient care and associated services within a defined specialty practice. The coordinator assesses patient/family needs, coordinates care and associated procedures or services for complex patients, and provides education to patients and families. They offer telephone advice based on protocol, acting as an advocate and resource for patients, families, and the health care team. This role also involves mentoring staff, delegating tasks, and collaborating with other healthcare providers, schools, and community services to meet patient/family needs.
