About The Position
The Clinical Care Coordinator, in alignment with the goals of PCMH (Patient Centered Medical Home), will be primarily responsible for panel management within primary care. The Clinical Care Coordinator is responsible for establishing and maintaining multiple working relationships with; providers, internal and external care teams, front office assistants and primary care core leaders to ensure optimal quality health care. Requirements
- National Provider BLS - American Heart Association Upon hire
- Washington Medical Assistant Registered Within 60 days of hire
- Office experience including, customer service, computers, Microsoft Office software (Excel, Publisher and Power Point), and patient interaction experience.
Nice To Haves
- 2 years Healthcare experience.
