About The Position

Journey Nursing Services aims to provide people-centered care with a holistic approach for both the patient and their care system. We are expanding and offer personalized and holistic private duty nursing, Nurse delegation, and Nursing pool services. As a newer company, we offer a great deal of growth and opportunity to our employees. This position is a split role, involving both administrative and caregiving responsibilities.

Requirements

  • High school graduate or G.E.D certificate.
  • CNA or HCA license to provide home care in Washington State.
  • At least 1-year experience as a Certified Nursing Assistant or Home Care Aide (in homes or facilities), or other applicable experience.
  • Technology Savvy.
  • Understanding of Microsoft Office: Word, Outlook, Excel.
  • Be listed in good standing with the Washington DSHS Background Check System.
  • Ability to read, write, speak, and understand English.
  • Have transportation to and from job.
  • Valid driver's license and car insurance.
  • Active CPR and First Aid or BLS card.

Nice To Haves

  • EMR experience a plus.
  • Population-specific experience a plus (e.g., dementia, Parkinson's, hospice/end-of-life care).
  • RN delegation certificate a plus.

Responsibilities

  • Support, lead, and train care givers in the field.
  • Assist in patient admission process and provide hands-on patient care at the place of assignment.
  • Conduct frequent visits to client homes for plan of care updates, staff training, compliance requirements, skill nursing visits, and shift coverage.
  • Recognize and address staffing difficulties proactively.
  • Work with the caregiving team and family to ensure appropriate and adequate staffing.
  • Investigate all unfilled shifts and take appropriate actions.
  • Serve as a backup trainer for offered courses.
  • Foster a high-performing culture of excellence that drives employee engagement and provides exceptionally reliable, safe, high-quality patient care.
  • Assist with administrative tasks, record keeping, and documentation.
  • Perform other job-related duties as needed.
  • Perform hands-on patient care in various settings (in-home, facility, or adult family home).
  • Provide quality care that promotes patient rights, dignity, freedom of choice, and individuality.
  • Encourage patients/clients to maintain or attain the highest practical physical, mental, and psychosocial well-being.
  • Ensure individualized care plans are followed and report any changes in condition or observations to a nurse supervisor.
  • Assist with Activities of Daily Living (ADLs) including grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, and other needs.
  • Promote skin integrity to prevent pressure ulcers and skin tears.
  • Treat patient/client personal property with respect.
  • Perform delegated tasks assigned by the nurse, including vital signs, resident weights, and application of creams/ointments.
  • Lift up to 50 lbs using proper body mechanics or lifting devices.
  • Follow safety and infection control procedures, including hand washing and proper disposal of materials.
