About The Position

The Clinical Care Coordinator LPN at Menders by Lenbrook LLC is responsible for overseeing and coordinating clinical care activities to ensure high-quality patient outcomes. This role involves collaborating with healthcare professionals, managing patient care plans, and ensuring compliance with healthcare regulations and standards.

Requirements

  • Licensed Practical Nurse (LPN) certification from an accredited nursing program
  • Minimum of 2 years of clinical nursing experience, preferably in a care coordination role
  • Experience working with geriatric populations or in a long-term care setting
  • Familiarity with electronic health records (EHR) systems
  • Strong organizational skills with the ability to manage multiple tasks and priorities effectively
  • Excellent communication skills, both verbal and written, with the ability to interact professionally with patients, families, and healthcare teams
  • Proficient in using computer applications and software related to healthcare management
  • Ability to work collaboratively in a team environment and demonstrate leadership in coordinating patient care
  • Knowledge of healthcare regulations and compliance standards relevant to patient care and coordination

Responsibilities

  • Coordinate and manage patient care plans in collaboration with healthcare providers to ensure effective and efficient delivery of clinical services.
  • Monitor patient progress and make necessary adjustments to care plans to optimize health outcomes.
  • Serve as a liaison between patients, families, and healthcare providers to facilitate communication and ensure patient needs are met.
  • Ensure compliance with healthcare regulations, policies, and procedures to maintain high standards of care.
  • Provide education and support to patients and families regarding health conditions, treatments, and care plans.
  • Assist in the development and implementation of clinical protocols and procedures to enhance service delivery.
  • Participate in quality improvement initiatives to continuously improve patient care and service efficiency.
  • Maintain accurate and up-to-date patient records and documentation in accordance with regulatory requirements.
  • Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care for patients.
