About The Position

The Oncology RN Clinical Care Coordinator (OCC) works collaboratively with an oncologist in delivering care to their assigned patient population. This is an outpatient position and requires oncology clinical experience and sound nursing knowledge related to the care of patients undergoing cancer treatment to assist with the education, the coordination of care, and ongoing assessment of the patient's response to treatment. The OCCC assesses patients for symptoms, clinical and psychological needs, treatment-related concerns, and barriers to care. Provides education regarding diagnosis, treatment, medications, side effects, and symptom management. Connects patients and families with appropriate resources as determined by the healthcare team. Functions as the primary clinical point of contact for hematology and oncology patients and follow them throughout the continuum of care. Provides telephone and MyChart triage, escalates concerns to the provider as appropriate, and support physicians and APNs with patient questions, clinical follow-up, coordination of care, and completion of disability and other treatment-related forms. Works collaboratively with the multidisciplinary oncology team to close the loop on outstanding test, referrals, and follow-up needs, and to prevent gaps or delays on care. Acts as a clinical expert and role model for their assigned disease team and participate in committee work, quality initiatives, and patient programs that advance the cancer program and resources available for patients.

Requirements

Minimum of a Bachelor of Science in Nursing (BSN) or a related healthcare field required.
Minimum of three (3) years of full-time experience in a hospital-based practice required, preferably in Oncology.
Current CPR certification required.
Oncology Nursing Society (ONS)/ONCC Provider Chemotherapy Biotherapy Certificate and/or Radiation Therapy Certificate required, or must be obtained within the first ninety (90) days of employment.
All required certifications must be maintained throughout employment.
Chemotherapy certification requirement remains as stated in the job description.

Nice To Haves

Master’s Degree in Nursing, Healthcare Administration, Public Health, or a related field preferred.
Spanish-speaking candidates are highly preferred.
Strong team leadership, communication, and interpersonal skills required.
Oncology Certified Nurse (OCN) national certification preferred.

Responsibilities

Collaborate closely with oncologists to provide comprehensive care for assigned hematology and oncology patients in an outpatient setting.
Serve as the primary clinical point of contact, supporting patients throughout the continuum of cancer care.
Conduct ongoing assessments of patient symptoms, treatment response, psychosocial needs, and barriers to care.
Educate patients and families on diagnoses, treatment plans, medications, side effects, and symptom management strategies.
Coordinate care across multidisciplinary teams to ensure timely follow-up, referrals, testing, and treatment interventions.
Connect patients and families with appropriate internal and community resources based on individual needs.
Provide telephone and MyChart triage, addressing patient concerns and escalating clinical issues to providers as appropriate.
Support physicians and Advanced Practice Nurses (APNs) with patient follow-up, care coordination, and completion of disability and treatment-related documentation.
Monitor and facilitate continuity of care to prevent gaps, delays, or obstacles in treatment.
Serve as a clinical resource and subject matter expert for the assigned disease team.
Participate in quality improvement initiatives, committee activities, and patient-centered programs that enhance oncology services and outcomes.
Apply advanced oncology nursing knowledge and clinical expertise to support patients undergoing cancer treatment and survivorship care
