About The Position

Merakey is seeking a Clinical Assessor (LPC/LCSW) to join their team in Hammond, LA. The Clinical Assessor will travel to various sites to conduct assessments with new and existing clients. Depending on the type of assessment, the employee will determine medical necessity for ACT services or nursing home placement. For follow-up hospitalization assessments, the employee will ensure the consumer has an appropriate and attainable discharge plan.

Requirements

• Master’s degree in human services
• LPC/LCSW required
• Valid driver's license required

Responsibilities

• Travel to various sites to conduct assessments with new and existing clients.
• Determine medical necessity for ACT services or nursing home placement.
• Ensure consumers have an appropriate and attainable discharge plan in place for follow-up hospitalization assessments.
