Requirements

  • Knowledge of DSM-5 and general clinical principles and practices.
  • Ability to work cohesively within the treatment team.
  • Excellent verbal and written communication skills and ability to interact with individuals of various social, cultural, and educational backgrounds.
  • Ability to understand and follow written and verbal instructions.
  • Excellent organizational skills and attention to detail.
  • Ability to exercise confidentiality, discretion, and good judgment.
  • Proficient with Microsoft Office Suite or related software.
  • Ability to read, write, and speak English; Spanish preferred.
  • High school diploma or high school equivalency diploma and a combination of education in a field related to behavioral health and full-time behavioral health work experience totaling at least two years
  • Associate's degree and at least two years of full-time behavioral health work experience
  • Bachelor’s or master’s degree plus one-year behavioral health experience OR Bachelor’s or master’s degree with a behavioral health focus
  • New Hires are required to pass pre-employment background check and drug testing (effective 11/1/2022).
  • Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire.
  • Basic Life Support (BLS) certification.

Nice To Haves

  • 1+ years of Clinical Assessment experience in outpatient behavioral health setting preferred.

Responsibilities

  • Conduct comprehensive intake assessments and complete all required documentation, including ROI, Consents, PHQ-9, ASAM, CALOCUS, and other relevant inventories for new and returning patients.
  • Collaborate closely with assigned providers, case managers, and internal and external patient teams to support caseload management, treatment plan and address identified patient needs.
  • Participate in staff meetings, treatment planning sessions, and case conferences to ensure coordinated care.
  • Complete and process Releases of Information (ROI) for prior outpatient and inpatient treatment services.
  • Provide outreach and engagement following internal and external crisis interventions.
  • Deliver supportive counseling services under the direct supervision of a licensed behavioral health professional.
  • Engage patients experiencing challenges with treatment adherence and assist in resolving barriers to appointment compliance.
  • Connect patients with appropriate community resources and document all interactions in accordance with NOAH policies and procedures.
  • Educate patients on available community services and facilitate appropriate linkages.
  • Contribute to quality improvement initiatives to enhance case management and provider service delivery.
  • Maintain strict adherence to patient confidentiality and rights in compliance with NOAH policies, contractual obligations, and state and federal regulations.
  • Consistently meet established productivity expectations for the position.
  • Perform other related duties as assigned.
