About The Position

Acentra Health is looking for a Clinical Assessor, Mid to join our growing team. The purpose of this position is to complete needs-based eligibility face to face assessments for patients who are applying for Medicaid-funded personal care services provided in their home or in adult care or supervised living homes. Clinical Assessors also complete face-to-face, clinical assessments of individuals with serious mental illness (SMI), intellectual disability (ID), or related conditions (RC) admitting into nursing homes. These preadmission screening and resident review (PASRR) Level II evaluations typically occur in nursing home or hospital settings. This role requires a valid driver's license and travel to various locations within New Hampshire.

Requirements

Registered Nurse or LCSW license issued from the State of New Hampshire
Resident of the State of New Hampshire
Strong typing and computer skills required with proficiency in Microsoft Word and Outlook.
Excellent clinical experience and knowledge of psychiatric disorders as defined by the most current Diagnostic and Statistical Manual in use in New Hampshire.
Knowledge of standards of practice related to mental health, intellectual and developmental disabilities, geriatrics, and other disorders typically requiring long term care services and supports.

Nice To Haves

Minimum of two years' experience in a home care setting preferred.

Responsibilities

Ensure accurate PASRR evaluations and recommendations through accurate clinical assessment of the patient, relevant records, and collateral sources of individuals.
Assess the appropriate level of care and service needs based on the patient’s physical, behavioral, and psychological functioning.
Complete medical eligibility assessments (MEA) on time and within scope of practice to meet all contractual requirements.
Ensure compliance with program workflows.
Ability to complete person-centered assessments with meaningful and appropriate recommendations for level of care.
Work collaboratively with healthcare professionals, individuals and families, and other community supports.
Communicate effectively with individuals and groups through spoken, written, and electronic media.
Ability to attend to detail, effectively prioritize, and execute tasks in a timely manner.
Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.
