About The Position
Join the CentraCare MTM Team and Make Rural Life Healthier! We are seeking a Clinical Ambulatory Pharmacist to support patients at the River Campus Nephrology. In this role, you will provide comprehensive medication management, ensuring therapies are indicated, safe, effective, and convenient for each patient. You’ll work directly with patients and collaborate with an interprofessional team to optimize outcomes. This position requires strong clinical expertise and the ability to build positive, trusting relationships with patients and colleagues across the care continuum.
Requirements
- PharmD degree and/or BS Pharm degree required.
- Current licensure by the Minnesota State Board of Pharmacy or eligibility for licensure required.
- Basic Life Support (BLS) Certification
Nice To Haves
- Completion of a PGY1 or PGY2 ambulatory care residency or comparable experience in ambulatory clinical pharmacy highly preferred
- Eligible for Pharmacy Specialty Board Certification preferred
