About The Position

The Clinic Social Worker (CSW) exercises independent judgment and professional discretion in the practice of case management to a selected group of patients. The CSW collaborates with the Mind Wellness Team and serves as advocate to help patients and families navigate care. The CSW is proactive and uses collaborative problem solving skills to meet the needs of the patients and Providers. The CSW provides social work case management services as needed and relies heavily on such resources as family, community, state and federal agencies as well as the patient’s resources to assist the patient’s and/or family’s return to a productive life.

Requirements

• Bachelor's degree is required.
• Social work licensure in the state of WV: LSW is required.
• Minimum of up to 1 Year of Position-Related Experience is required.
• Social work experience is required.
• Excellent verbal and written communication skills.
• Ability to build effective relationships with the healthcare team.
• Demonstrate critical thinking skills.

Nice To Haves

• 2 to 5 Years of Position-Related Experience is preferred.
• Experience in the medical field is preferred.

Responsibilities
