About The Position
TinkBird Healthcare Staffing is seeking an experienced Registered Nurse (RN) Clinic Nurse for a full-time permanent opportunity with a PACE (Program of All-Inclusive Care for the Elderly) organization in Winston-Salem, North Carolina. This position provides direct and indirect nursing care to frail and elderly participants while working as part of an interdisciplinary team focused on improving quality of life and supporting participants in maintaining independence. The practice environment is a PACE setting with a focus on caring for frail and elderly participants, utilizing a collaborative interdisciplinary care model. This role offers the opportunity to build long-term relationships with participants and families, with an emphasis on preventative care, chronic disease management, and quality outcomes.
Requirements
- Graduate of an accredited School of Nursing
- Active North Carolina RN license required
- Valid North Carolina driver's license and reliable transportation required
- Minimum of 3 years of RN experience required
- At least 24 months of experience working with aging, frail, or elderly populations required
- Strong assessment, care coordination, and clinical decision-making skills
- Ability to work independently and within a multidisciplinary team environment
Responsibilities
- Perform comprehensive nursing assessments and ongoing participant evaluations
- Develop, implement, and revise individualized care plans
- Coordinate care with physicians, specialists, caregivers, and interdisciplinary team members
- Monitor participant health status and communicate changes to the care team
- Provide participant and family education regarding health conditions and treatment plans
- Administer treatments and nursing interventions according to established standards of care
- Complete required documentation accurately and timely
- Participate in interdisciplinary team meetings and care planning
- Assist with emergency situations and initiate appropriate nursing interventions
- Serve as a resource and leader for clinical staff and support personnel
