About The Position

Critical Care, Pulmonary and Sleep Associates is a multispecialty private physician practice, providing Critical Care, Pulmonary and Sleep medical services to the Denver metro area for over 40 years. Our dedicated team of over 40 Board Certified Critical Care, Pulmonary and Sleep Physicians along with our skilled clinical and administrative staff exclusively serve nine Common Spirit and AdventHealth Hospital ICU’s and multiple outpatient clinic locations spanning Colorado’s front range. We are currently seeking a Clinic Nurse to join our committed team of professionals. The Clinic Nurse position is an essential clinical role, providing onsite clinical support to multiple Pulmonary and Sleep clinic’s day to day operations. The Clinic Nurse must hold an RN degree, BSN preferred and have experience within the nursing field. Working in conjunction with the RN team and clinic staff, they will assist in patient call triaging, medication management, RN visits, care coordination as well as other operational and clinical duties. It is necessary that within this role, the Clinic Nurse cultivates and supports a strong team culture and collaborative environment centered on consistently delivering exceptional patient care.

Requirements

  • RN degree required, BSN preferred.
  • Minimum of 1-3 years’ experience in the nursing / healthcare field.
  • Strong clinical knowledge, ability to assess and triage patients over the phone and in person.
  • Demonstrate knowledge of creating excellent patient experiences in an outpatient clinic setting.
  • Superior communication skills to effectively and professionally communicate via email, telephone and in person with patients, Providers and staff.
  • Respect patient privacy by maintaining confidentiality in accordance to HIPAA requirements.
  • Excellent organizational and time management skills.
  • Computer proficiency, knowledge of EMR systems.

Responsibilities

  • Promote the CCPSA culture of team collaboration, while providing excellent clinical care to our patients.
  • Triage patient calls and elevate to APP team or MD as needed.
  • Schedule audio visits as needed for certain triage patients needing priority assistance and APP advise
  • Schedule established patient follow up appointments as needed
  • Assist in the coordination and management of our Lung Health Initiative program
  • Build strong positive communication with clinic staff to increase the success of the practice and provide excellent patient care.
  • See patients during nurse visits to assess patient needs.
  • In collaboration with the Providers, develop patient education on medications and disease states.
  • Build strong positive working relationships and communication with Providers.
  • Manage medications for patients; including education, prior authorizations when applicable, and necessary monitoring.
  • Conduct care coordination for our patients.
  • Conduct care coordination and evaluation process for Endobronchial Valve candidates in conjunction with our IP physicians and care team
