About The Position

This role involves providing nursing care within a clinic setting, with responsibilities split between phone-based patient assessment and direct patient care as a clinic float. The position requires a strong understanding of clinical functions, patient assessment, and effective communication. The nurse will be responsible for documenting patient interactions, administering medications, preparing patients for examinations, assisting with procedures, and maintaining clinic supplies and equipment. The role emphasizes patient safety, adherence to protocols, and professional development.

Requirements

  • Graduate of an approved school of nursing
  • LPN or RN Licensure in the State of Indiana
  • CPR certification
  • Knowledge of clinical function
  • Ability to work in conjunction with other clinic personnel
  • Knowledge of community health resources for alternate health services
  • Knowledge of community health nursing principles and practices
  • Ability to communicate effectively both verbally and in written reports
  • Demonstrate excellent communication skills and empathy towards patients
  • Professional demeanor and ability to handle stressful situations calmly
  • Follows the five rights of medication administration to reduce the potential for error
  • Formulates a teaching plan based on identified learning needs and involves patient and family in education

Nice To Haves

  • Two years’ experience in physician’s office or related experience
  • Experience in Urology preferred
  • Background in medical-surgical nursing and immunizations preferred

Responsibilities

  • Conducting patient assessments over the phone by asking detailed questions about symptoms & medical history.
  • Utilizing critical thinking skills to determine the appropriate level of care needed.
  • Providing medical advice based on the assessment.
  • Directing patients to the right healthcare provider or service.
  • Maintaining accurate documentation of each call.
  • Updating the patient’s medical and social history, as appropriate.
  • Maintaining current knowledge of medications and their correct administration.
  • Accurately documenting all patient interactions including symptoms, advice given, and follow up plans in the electronic medical record.
  • Scheduling patients for further testing as directed by the Provider.
  • Securing appointment date and time from specialty referral as directed by the Provider.
  • Sending in prescription refills as directed by the Practitioner.
  • Escorting patients to exam rooms and assessing patients per clinic protocols.
  • Obtaining and thoroughly documenting vital signs, chief complaint.
  • Preparing patient for examination and assisting as necessary.
  • Ensuring that ordered tests results are in the patient record and available for Practitioner review at time of patient examination.
  • Assuring adequate patient records are maintained and transferred as requested.
  • Obtaining consent for procedures as directed.
  • Collecting lab specimens and preparing specimens for transport according to policy.
  • Assisting provider in procedures including but not limited to dressing changes, pelvic exams, laceration repairs, injections, and aspirations.
  • Utilizing appropriate aseptic technique.
  • Cleaning instruments and preparing them for sterilization according to policy.
  • Formulating a teaching plan based on identified learning needs and involving patient and family in education.
  • Keeping track of follow-up labs, wellness checks for future patient reminders.
  • Responding to emergency situations appropriately and immediately.
  • Calling 911 as necessary.
  • Records all phone conversations and information given to the patient in the medical record to provide for clear communication among all clinic staff.
  • Communicating patient concerns and relevant information to the appropriate healthcare provider for further evaluation or treatment.
  • Processing prescription refills as directed by the provider.
  • Contacting patients to discuss the results of laboratory test or imaging studies.
  • Identifying situations that require immediate medical attention and directing patients to the appropriate emergency care facility.
  • Participating in professional development and assuming responsibility for keeping knowledge of skills current.
  • During downtime utilizing time wisely by assisting with clinic buckets in the EMR.
  • Ensuring that exam rooms and patient accessible areas are safe and that likelihood of patient injury is reduced.
  • Assuring that examination rooms are always full and that patients are seen in an orderly and fair manner.
  • Ensuring that acutely ill or injured patients are triaged and seen first.
  • Explaining delays and waiting times to patients and families.
  • Maintaining knowledge of lab collection policy and procedures.
  • Performing quality controls for tests and documenting results as per policy.
  • Monitoring and recording oxygen tanks monthly.
  • Checking AED monthly. Records activity.
  • Performing API QC during the year on all tests resulted at site.
  • Checking for outdates on supplies and medications on an ongoing basis.
  • Inventoring medications upon receiving and disposing. Inventories, orders and manages the VFC medications.
  • Answering telephone calls in a professional manner and referring patient problems to Practitioner.
  • Interacting professionally with patients and families.
  • Exhibiting ability to prioritize and organize work.
  • Following through on assigned tasks.
  • Participating in departmental performance improvement activities by collecting, analyzing, and reporting data.
  • Knowledgeable of and compliant with clinic policies and procedures.
  • Exhibiting cooperation and positive attitude and maintaining a good working relationship with all clinic staff.
  • Completing incident reports and forwarding to Practitioner when appropriate.
  • Completing routine equipment checks and stock supplies.
  • Ordering supplies as needed.
  • Monitoring, ordering, and stocking exam rooms with supplies as needed.
  • Performing other duties as assigned.
