About The Position

The Clinic Manager PACE is responsible for daily oversight of clinic operations and the supervision of the Registered Nurses and Patient Care Assistants. The Nurse Practitioners do have a Collaborative Agreement with the PACE Medical Director; while the Clinic Manager PACE provides administrative functions. Requirements

  • Associates Degree in Nursing/RN - Required
  • 3 years Nursing/Patient Care - Required
  • 1 year Supervisory - Required
  • 1 year Work with frail/elderly - Required
  • Basic Life Support Program (BLS) - American Heart Association - Required
  • Registered Nurse (RN) - State Licensing Board - Required
  • Drivers License - Required

Nice To Haves

  • Bachelor's Degree Nursing / Patient Care - Highly Preferred
  • 1 year PACE experience - Preferred
  • 1 year Ambulatory/Home Health - Preferred

Responsibilities

  • Ensure all clinic staff is practicing within their licensure, while also certifying all licensed clinical staff is current with licensure and compliant; ensure sufficient Day Room staffing by PCAs and Clinic per staffing guidelines.
  • Communicate with the Center Manager on case management/homecare operations and issues.
  • Support clinic nurses with continuous process improvement for nursing plan and protocols utilizing EPIC, while reviewing the plans of other disciplines to collaboratively meet the participants’ continuing care needs.
  • Demonstrate competency in clinical skills such as, physical assessment, intravenous therapies, symptom management, controlled substance management and participant relationships.
  • Consult, refer, and collaborate with other interdisciplinary team members or health care professionals to promote quality and continuity of participant care.
  • Apprise IDT of changes in participant’s condition, care needs, and/or concerns of participants.
  • Lead Polypharmacy calls with Providers and Contracted Providers.
  • Regularly communicates with nursing administration of contracted ALF and ECF facilities with participant updates and discusses any participant/staff issues requiring interventions by facility.
