About The Position

The Clinic LPN/CMA provides direct and indirect patient care under the direction of the Clinical Nurse Supervisor. This role is responsible for pleasant and effective communication with the public, promoting a positive office image. The position involves rendering basic supervised nursing care to patients, maintaining accurate electronic health records, and establishing quality, confidential patient care.

Requirements

  • Current licensure as an LPN or CMA in the State of Idaho.
  • Must be able to pass any and all background checks required by law.
  • Demonstrates evidence of essential leadership, communication, education, collaboration, and counseling skills.
  • Proficient in communication technologies (email, cell phone, etc.).
  • 1 year of CMA or related experience.

Nice To Haves

  • Bilingual (English and Spanish).
  • 3-5 years’ experience in clinical or community health settings preferred.
  • Previous experience with health IT systems and data reports preferred.

Responsibilities

  • Room patients promptly and efficiently.
  • Implement an effective internal tracking system for identified patients.
  • Coach patients/families toward successful self-management of their diagnosis.
  • Utilize tools and documents that support a guided care process, collaborate with patient/family toward an effective plan of care.
  • Assess patient and family’s unmet health and social needs.
  • Provide effective communications to improve health literacy.
  • Develop a care plan based on mutual goals with the patient, provider, and family.
  • Create ongoing processes for patients/families to determine and request the level of care coordination support they desire over time.
  • Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support specialists (e.g., Diabetes Educator).
  • Cultivate and support primary care and subspecialty co-management with timely communication, inquiry, follow-up, and integration of information into the care plan regarding transitions-in-care and referrals.
  • Serve as the contact-point, advocate, and informational resource for patient, family, care team, payers, and community resources.
  • Ensure effective tracking of test results, medication management, and adherence to follow-up appointments.
  • Facilitate and attend meetings between patient, families, care team, payers, and community resources, as needed.
  • Demonstrate a positive, respectful attitude and professional customer service.
  • Acknowledge patients’ rights on confidentiality issues, maintain patient confidentiality at all times, and adhere to HIPAA guidelines and regulations.
  • Demonstrate continual learning skills, effect changes in approach to care based on established, evidence-based practice.
  • Cultivate effective partnerships, effectively collaborate with all practice providers (Physician, Nurse Practitioner, Physician Assistant and other licensed allied health team-members).
  • Demonstrate understanding in use of IT resources and patient databases.
  • Demonstrate effective delegation skills to streamline operational workflows and optimize inter-office resources.
  • Support and maintain a culture of safety and quality.
  • Carry out medical and surgical asepsis during treatments and special procedures.
  • Meet current documentation standards and policies.
  • Assist in answering clinic telephone calls in a professional manner; relay patient information and concerns to attending physician; assist other staff in scheduling patient appointments.
  • Ensure an adequate stock of supplies and proper functioning of equipment.
  • Demonstrate the ability to be flexible, organized and function under stressful situations.
  • Actively engage in quality improvement activities.
  • Perform other duties as assigned within the LPN / CMA scope of practice.
