The Client Coordinator is responsible for ensuring the overall operations and services re being delivered according to state and federal regulations, agency policy and procedure and according to the agency’s mission. The Client Coordinator serves to provide leadership, direction, and training in order to supervise the overall development of a specific program. Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, develop implementation strategies which outline services and supports for individuals so they may work towards the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. The Client Coordinator is responsible for ensuring service provision by all disciplines represented on the treatment team. The Client Coordinator meets routinely with other Team Clinicians for coordination and integration of services. The Client Coordinator coordinates with the Program Coordinator(s) and staff and the provision of individual and day program services as well as the maintenance of a therapeutic milieu which is conductive to participants’ health and well-being. The Client Coordinator coordinates and oversees the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department and completion of a required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. The Client Coordinator accepts primary responsibility for development of program goals and methodologies and for supporting the DSPs to correctly implement and document such activities. The Client Coordinator is primarily responsible for ensuring the goals and activities are developmentally appropriate and are in response to identified prioritized needs, and to relay relevant information to the team. . The Client Coordinator ensures provision of integrated services to individuals by fostering within each Team an awareness of the need for service integration, coordinating service delivery and planning activities with day program services, monitoring service provision by agency clinical staff, consultants and community based vendors, and ensuring services are in accordance with IDT treatment recommendations. The Client Coordinator ensures provision of a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to ensure provision of coverage levels consistent with the treatment needs of the individuals and regular monitoring of the day program setting to ensure maintenance of an atmosphere conducive to individuals' development and well-being, where the rights of each individual are upheld and respected. The Client Coordinator regularly records progress of individuals and documents all changes in Plans of Care / ISPs according to procedures established by the agency. The Client Coordinator reports regularly to the Supervisor on issues of program compliance, noncompliance, and remedial actions taken to correct deficiencies. The Client Coordinator identifies needs and requests supports, assistance or direction to meet service needs and ensure program compliance. The Client Coordinator participates in clinical reviews, incident review and human rights committee meetings and other meetings as directed by Supervisor. As schedule allows, the Client Coordinator is the primary chair on behalf of the agency in the Life Plan Meetings. The Client Coordinator reviews documentation and provides technical assistance or direction to program management and staff.
