About The Position

Visiting Angels of Auburn is hiring a Client Case Manager! This is a full-time position that will work in our Auburn Office. On-call assignments will be needed as well which offers an incentive pay. This position assists with marketing and attaining new clients, conducting Care Coordination meetings, managing client cases and maintaining strong relationships with them, working with our Caregivers to properly staff cases, performing field evaluations, and other assigned tasks. In this role, you will be able to have a direct impact in allowing our clients to stay in their homes. Our Case Managers help to acquire and onboard new clients, schedule shifts, coach Caregivers, and make a difference each and every day. This role will require both office work and field work--meeting with potential clients, conducting field visits with clients and Caregivers, and possibly working shifts at times if needed.

Requirements

  • Associate Degree
  • Demonstrate proficiency with Microsoft Office (Word, Excel and Outlook) applications, scheduling systems and other health care industry related software.
  • Ability to listen and communicate clearly, fluently and diplomatically orally and in writing.
  • Ability to remain flexible, resilient, calm and maintain a sense of humor; and present a well-groomed professional image.
  • Ability to plan, organize, prioritize, delegate and accurately follow through in work activities with time constraints and interruptions to meet deadlines, as well as, work independently with a minimum amount of direction and/or supervision.
  • Ability to generate goodwill for the Agency with staff, clients, their family members and referral sources.
  • Demonstrate a strong commitment to client service excellence.
  • Ability to lawfully work in the U.S.

Nice To Haves

  • Two years of supervisory experience in home care is preferred.
  • Experience is preferred, though we are willing to train the right person.
  • Great role for someone who has LNA/Case Manager experience and looking to grow in their skills.
