About The Position

Freedom Home Care is seeking a highly organized, compassionate, and service-driven Client Care Coordinator to join their team at their Mankato headquarters. This role serves as the operational heartbeat of the care team, coordinating caregiver schedules, maintaining accurate client and employee data, and acting as a key liaison between clients, families, and caregivers. Requirements

  • High school diploma or equivalent required.
  • Proficient in Microsoft Office Suite and comfortable using scheduling platforms.
  • Excellent verbal and written communication skills.

  • Flexibility and resilience when responding to unexpected scheduling changes and urgent needs.

Responsibilities

  • Develop and manage caregiver schedules for new and existing clients, ensuring consistent, high-quality coverage.
  • Monitor and adjust daily schedules in real time to accommodate changes, call-offs, and urgent needs.
  • Serve as the primary point of contact for day-to-day scheduling communications with clients and caregivers.
  • Maintain scheduling accuracy in the home care software system (Axis Care), including caregiver logs, shifts, and client profiles.
  • Ensure timely system updates to support accurate payroll and billing.
  • Participate in the after-hours/on-call rotation to provide scheduling support outside of business hours as needed.
  • Assist with general office operations, including answering phones, greeting visitors, and supporting administrative projects.
  • Build and maintain positive, professional relationships with caregivers, clients, and families.
  • Provide support and encouragement to caregivers while promoting reliability, accountability, and compassion.
  • Actively contribute to a collaborative, mission-driven, and respectful team culture.
