The Client Care Coordinator will be responsible for overseeing client care services, conducting consultations, supporting caregivers, and ensuring the highest level of client satisfaction. Requirements

  • Nursing designation (LPN or RN) is required
  • Experience in home care, senior care, or case management is required
  • Strong communication, problem-solving, and organizational skills
  • Ability to work independently and as part of a team
  • Valid driver’s license and access to a reliable vehicle
  • Proficient in basic computer systems and documentation

Responsibilities

  • Conduct in-home consultations with prospective clients and families
  • Assess client needs and develop personalized care plans
  • Facilitate client and caregiver introductions to ensure smooth onboarding
  • Build and maintain strong relationships with clients and their families
  • Ensure ongoing client satisfaction and retention
  • Identify opportunities to enhance or adjust service plans through regular follow-ups to better meet clients’ evolving needs.
  • Conduct regular quality assurance visits and check-ins
  • Evaluate and update care plans based on client needs and feedback
  • Address and resolve client concerns or service issues promptly
  • Ensure all services meet company standards and compliance requirements
  • Collaborate with caregivers to ensure proper delivery of care
  • Participate in caregiver performance discussions and coaching when needed
  • Foster a positive, supportive, and team-oriented work environment
  • Maintain accurate and up-to-date client care plan and records in the system
  • Coordinate care schedules in collaboration with the operations team
  • Participate in team meetings and contribute to continuous improvement
  • Ensure confidentiality and professionalism in all interactions
  • Maintain clear, open, and effective communication with the Director of Care and team members.
