As an integral member of the healthcare team, the Certified Care Attendant (CCA) fulfills assigned responsibilities related to patients' daily routines and fundamental care requirements. Working within the framework of hospital guidelines, protocols, and the unit's care delivery approach, the CCA executes tasks under the supervision of the Registered Nurse, ensuring patients' well-being and comfort. UHN's Reactivation Care Centre (RCC) provides a supportive space, for specialized care, focused on reactivation, for patients who no longer need acute care and are focused on the goal of safely returning to the community – whether that be home, long-term-care, or an alternative home setting. As part of the Ministry of Health and Ontario Health's commitment to alleviating hospital pressures across the Greater Toronto Area, the RCC, which UHN operates in partnership with Sinai Health and Trillium Health Partners, connects patients with the right care, in the right place, at the right time. Enhancing the Patient Experience UHN's RCC is nestled in a residential neighbourhood, where quiet surroundings and green space extend our calm and healing environment. While therapeutic programming keeps patients focused on achieving their individual reactivation goals – whether it's building strength and endurance, managing medication, or maintaining mobility, as they plan for a safe transition – our recreational schedule builds community, encourages socialization and connects patients to activities that are meaningful in their everyday lives.
