About The Position

NATURE OF WORK: This position serves as Assistant Manager, working under the general supervision of the Client Advocacy Services Manager in the area of Waiver and Guardianship programs within the Office on Aging. This position is on call 24 hours a day, 7 days per week to respond by phone and, as needed, in person to emergency concerns related to vulnerable adults who are court-appointed into our Guardianship program. Position is responsible for case management services and makes long-term and daily (as needed) well-being decisions for vulnerable adults. Position completes screening/assessment of needs of elderly wards, facilitates placement, and attends medical and/or psychiatric care appointments with clients to facilitate and approve medical care and procedures. Coordinates as needed with other agencies, including the courts, DSS, the hospital, care providers in residential facilities, attends court proceedings and represents (with support of County legal counsel) case management concerns. Completes status updates to the courts, relevant and timely documentation, and administrative functions, including data collection, data analysis, and report writing.

Requirements

  • Knowledge of the elderly and their needs
  • Skilled in counseling and interviewing techniques
  • Skilled at performing social case work
  • Ability to supervise, to plan, direct, and evaluate care, to prepare reports
  • Ability to communicate effectively both orally and in writing
  • Knowledge of basic computer programs, including Word and Excel
  • Ability to learn new computer programs as required by the State
  • Ability to drive a county vehicle to conduct field work
  • Ability to climb stairs, walk, and lift or move objects of lightweight.
  • Graduation from an accredited college or university with a Bachelor's degree majoring in one of the following human service programs: Gerontology, Social Work, Counseling, Psychology, Nursing, Vocational Rehabilitation, Sociology, or Social Science, or relative to a Human Services degree.
  • Six years of responsible case management experience in a human services or a field closely related to aging programs and services.
  • Possession of a valid Class "C" non-commercial driver's license with no more than three (3) points.
  • Must comply with all Federal (OSHA), State of Maryland (MOSH), safety regulations and applicable Harford County safety policies, procedures, or manual as required for this position.

Responsibilities

  • Coordinates client supports for agency wards, including placement, ongoing care, and medical treatment.
  • Performs intensive social case work, including screening and/or assessing the ongoing needs of wards and facilitating services/care to meet those needs.
  • Assists clients in preparing applications for services.
  • Maintains timely documentation, data, and case notes on an ongoing basis.
