About The Position

New York Cancer & Blood Specialists (NYCBS) is seeking a dedicated CNA/Medical Assistant to join their Chronic Care Management team in Bohemia, NY. This role involves introducing the Chronic Care Management (CCM) program to eligible patients, obtaining consent, and documenting this in the EMR system. The position requires collaboration with the CCM Supervisor to review care plans, advocate for patients, and facilitate communication between providers and patients. The successful candidate will also be responsible for documenting all activities in the EMR system.

Requirements

• High School Diploma or equivalent
• Certified CNA, or Medical Assistant
• 2+ years of applicable experience
• Proficiency in Word/Excel

• Bilingual, English/Spanish a plus

Responsibilities

• Introduce the CCM program to eligible patients via telephone.
• Obtain verbal consent and provide appropriate documentation in the EMR system.
• Under direct supervision of the CCM Supervisor, review and monitor documents related to the patient’s individualized care plan.
• Advocate for patients, collaborate with other wellness services at NYCBS, and provide educational information.
• Facilitate the transmission of information between providers and patients, as well as access to services needed.
• Document all activities in the EMR system.
