About The Position

The Health Home Lead Care Manager oversees the delivery of comprehensive care management services while also providing guidance, mentoring and supervision to care management staff. The Lead Care Manager also conducts and schedules assessments, referrals, advocacy and supports, counseling, education of patients and enrollees and care team members assuring the patient receives quality services to maintain optimum healthcare needs without barriers. The Lead Health Home Care Manager participates in outreach and engagement to formally enroll referred individuals into the care management program. The Lead Health Home Care Manager provides supervision to care managers to include on-boarding, training, monthly case reviews and auditing.

Requirements

  • A Bachelor’s degree with three (3) years relevant case management experience, or NYS licensure and registration as a Registered Nurse, or A Master’s degree with one (1) year relevant experience.
  • Experience working with high-needs population including children with complex medical and/or behavioral needs.
  • Must have strong written, verbal and organizational skills.
  • Computer skills necessary.
  • Travels within tri-county frequently.
  • Authorized to work in the United States.

Nice To Haves

  • At least 1 year of supervisory experience preferred.

Responsibilities

  • Maintain and manage a caseload of approximately 20 acuity points.
  • Provide leadership, guidance, and supervision to care management staff, including onboarding, training and ongoing coaching.
  • Ensure compliance with Health Home program requirements, including timely documentation, assessments and care plan updates.
  • Conduct monthly case reviews with care managers for regulatory compliance.
  • Serve as an expert and resource for care managers regarding care management practices and community resources.
  • Monitor caseloads as it pertains to productivity, quality and performance.
  • Collaborate with Special Education Coordinator on program development and barriers associated with the delivery of care management services to assist in quality improvement.
  • Track and report audit results and complete improvement plans.
  • Maintains current care management documentation and information regarding care management activities within the required health information technology (HIT) system.
  • Attend Quarterly Regional meetings for the CNYHHN.
  • Conduct weekly staff meetings to discuss referrals, regulations and recent audit findings.
  • Participate in monthly conference calls with CNYHHN providers.
  • Will ensure compliance with the Central New York Health Home Network (CNYHHN) policies and procedures.
  • Will attend all required online and in-person training required by (CNYHHN) and agency.
  • Participate with agency committee(s) as assigned.
  • Attend and satisfactorily complete all required in-services.
  • Provide rotating after-hours on-call coverage outside of regular working hours with additional compensation in accordance with organizational policy.
