About The Position

EMPACT- Suicide Prevention Center, part of the La Frontera family, is seeking a Children's Case Manager for their San Tan location. This is a full-time position that requires flexibility in scheduling to meet client and family needs. The role involves providing direct service crisis intervention, support, transportation, and education to families and children, either independently or in conjunction with other in-home client services. Responsibilities include completing family needs assessments, making appropriate referrals to collaborating partners and community services, ensuring continuity of care, and assisting in coordinating communications with other agency departments or collaborators.

Requirements

Meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
Experience with the Child and Family Team process.
Fingerprint clearance card or eligibility to obtain one.
Valid AZ driver's license with no major infractions in the past three years.
At least 21 years of age at time of employment.
Demonstrated ability to remain calm in crisis situations.
Demonstrated ability to work independently and cooperatively.
Demonstrated ability to recognize personal limitations.
Demonstrated ability to relate positively and with empathy to all persons regardless of age, race, creed, gender or sexual orientation.
Maintain professional conduct consistent with ethical professional practice and the policies and procedures of the agency.
Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners.
If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required.
At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
All employees will have a minimum of a high school diploma or equivalent.

Bi-lingual (Spanish) is preferred.
