About The Position

As a Children’s Intervention Specialist Case Manager, you will be working in the community to help provide stabilization and support services to youth and their families during crisis situations. Salary: $22.28/hr. Schedule: Five days a week, in office, with three days starting at 8:00a to 4:30p and two days from 9:30a to 6p. Exact schedule to be determined upon conclusion of training period. 30 minute unpaid lunch. Competencies: The ideal candidate must be able to demonstrate the ability to provide supportive services for youth and their families. Must demonstrate the ability to maintain contact with the youth, family and their support system. Must demonstrate the ability to produce a progress note in the Cyber and NetSmart EHR. Must demonstrate the ability to generate and maintain appropriate documentation as noted by chart review with at least 98% accuracy. Must demonstrate the ability to access local networks of services as dictated by the current ICP. Must demonstrate the ability to maintain a professionalism in all interactions they have with providers, families, co-works, etc. Must participate in and pass all company mandatory training, including CSOC and Rutgers, as scheduled. Must demonstrate basic computer knowledge.

Requirements

  • Bachelor's degree (BA/BS) in a behavioral health or related human services field, such as social work, counseling or psychology and at least four to six months of experience working in human services or related field
  • Valid Driver's License required.
  • All positions require a valid driver’s license in good standing and pre-employment drug screening.

Responsibilities

  • Receive referrals from the CSA through a “warm line” at the office.
  • Diffusing difficult situations when engaging with families over the phone.
  • Provide information to families as needed on the CMR model and functions.
  • Assists with callbacks for delay dispatches to help assist with reduction of cancellations
  • Have the ability to work with the Supervisor and other team members to coordinate care for all clients served.
  • Maintain documentation with 98% accuracy, and within the required time frame
  • Assists dispatchers with scanning forms and uploading to records when needed
  • Help outreach to families for follow up with consumer satisfaction surveys and collect feedback
  • Assist with creation, organization, and building of dispatch folders for staff.
  • Participate in team meetings and staff meetings.
  • Attend all required trainings as scheduled.
