About The Position

Intensive In-Home Qualified Professional (QP) Pathways to Life, Inc. Location: 2808 S. Croatan Highway, Suite D, Nags Head, NC 27959 Schedule: Full-time, with a flexible schedule based on member and family needs Work Setting: Members’ homes, schools, community settings, and the Pathways to Life office Position Overview Pathways to Life, Inc. is seeking a compassionate and dependable Qualified Professional (QP) to join our Intensive In-Home Services team in Nags Head. The IIH Qualified Professional provides family-focused therapeutic interventions to children and adolescents experiencing serious emotional or behavioral challenges. Services are provided primarily in members’ homes and community settings and are designed to stabilize the family, improve daily functioning, strengthen relationships, and reduce the risk of hospitalization or placement outside the home. The ideal candidate is flexible, organized, comfortable working with families in their natural environments, and able to respond appropriately during crisis situations.

Requirements

• Bachelor’s degree in a human-services field with at least two years of documented experience working with the population served.
• Bachelor’s degree in a field other than human services with at least four years of documented experience working with the population served.
• Master’s degree in a human-services field with at least one year of documented experience working with the population served.
• Master’s degree in a field other than human services with at least two years of documented experience working with the population served.
• Qualifying experience should include work with children or adolescents experiencing mental health, substance use, developmental, emotional, or behavioral needs.
• Possess a valid driver’s license and reliable transportation.
• Be able to travel regularly throughout the assigned service area.
• Successfully complete and maintain required CPR and Nonviolent Crisis Intervention certifications.
• Complete all required agency and IIH model training.
• Be able to regularly walk, stand, or stoop.
• Be able to occasionally lift, carry, push, or pull up to 25 pounds.
• Maintain a flexible schedule to meet the needs of members and families, including evening availability and occasional weekend hours.

Nice To Haves

• Previous experience providing Intensive In-Home, community-based, or family-centered behavioral health services.
• Experience working with children, adolescents, and families experiencing significant behavioral or emotional challenges.
• Familiarity with Person-Centered Plans, Crisis Plans, NCTOPPS, and Treatment Authorization Requests.
• Experience using an electronic health record system to complete behavioral health documentation.
• Knowledge of crisis intervention, care coordination, and community resources.
• Experience working with schools, child welfare agencies, juvenile justice, or other child-serving systems.
• Compassionate, patient, and respectful of family values and cultural differences.
• Comfortable providing services in members’ homes and other community settings.
• Able to build rapport with children, adolescents, parents, and caregivers.
• Calm and capable of following established procedures during crisis situations.
• Dependable and consistent with appointments, documentation, and follow-up.
• Organized and able to manage a flexible community-based schedule.
• Comfortable working independently while remaining actively engaged with the IIH team.
• Able to maintain appropriate professional boundaries.
• Responsive to supervision, feedback, and ongoing professional development.

Responsibilities

• Provide face-to-face, family-focused therapeutic interventions in members’ homes, schools, and community settings.
• Help children, adolescents, and families develop coping, communication, problem-solving, parenting, and behavior-management skills.
• Provide psychoeducation to help members and families understand diagnoses, symptoms, treatment recommendations, and effective coping strategies.
• Deliver interventions consistent with the member’s Person-Centered Plan and the Intensive In-Home service model.
• Participate in the development and ongoing revision of Person-Centered Plans and individualized Crisis Plans.
• Assist with implementing crisis-prevention and stabilization strategies.
• Respond appropriately during behavioral health crises and follow established safety and crisis-response procedures.
• Monitor member progress and document the effectiveness of interventions.
• Complete accurate service notes and other required documentation within agency timelines.
• Assist with Treatment Authorization Requests, NCTOPPS interviews, assessments, and other clinical documentation as assigned.
• Coordinate with schools, medical providers, behavioral health professionals, child-serving agencies, and other community resources.
• Identify and engage natural supports that can help the member and family maintain long-term stability.
• Participate in weekly team meetings and collaborate with the Licensed Team Lead, other IIH team members, and external providers.
• Support members and families during transitions and participate in discharge planning.
• Attend individual and group supervision and complete required clinical training.
• Maintain member confidentiality and comply with HIPAA, agency policies, payer requirements, and applicable service standards.
• Participate in quality-assurance, compliance, and accreditation activities.
• Maintain a professional and respectful approach with members, families, coworkers, and community partners.
