About The Position
The Blended Case Manager promotes resiliency and recovery for children, adolescents, adults, and their families by accessing, coordinating, mentoring, and advocating for community resources and services. The position is located in Scranton, Pennsylvania, and requires 40 hours of work per week, a flexible schedule, and on-call availability.
Requirements
- Bachelor’s degree in Human Services or a related field.
- Work 40 hours per week.
- Have a flexible schedule.
- On-call availability is required.
