About The Position

myPlace Health is seeking a Chief Medical Officer (CMO) to play an integral leadership role for interdisciplinary care teams, aiming to enable seniors to live independent lives. The CMO will oversee care teams across an assigned region, ensuring high-quality care and an outstanding participant experience. This role is responsible for the oversight of clinical operations, health outcomes, and quality results within the assigned market(s). The CMO will lead the delivery and quality of participant care, clinical outcomes, and the implementation and oversight of the quality improvement program. Responsibilities include direct patient care, coaching IDT team members, and engaging with provider network partners, community-based organizations, and corporate sponsors. The position also involves actively supporting SCAN's vision and goals, with potential for other duties as assigned.

Requirements

  • MD or DO required; active board certification in geriatrics, complex care, home-based care, palliative care, internal medicine, or other related clinical area (with continued certification throughout employment)
  • Previous experience in a PACE model or similar capitated, value-based care, or integrated care environment
  • Ten or more years practice of medicine with a minimum of five years working with an elderly or frail population
  • Current, unrestricted medical license in the appropriate state(s) assigned
  • Current DEA certificate with continued licensure throughout employment
  • Up to date immunizations records

Responsibilities

  • Serve as the accountable Chief clinical executive that owns the overall quality and health outcomes of the assigned region while ensuring that we operate with high fidelity to our model of care
  • Oversee the regional development and implementation of the Quality Improvement and Performance Improvement Program (QAPI) in collaboration with other key leaders and staff assigned
  • Monitor regional clinical utilization trends and guide teams to create appropriate intervention strategies
  • Collaborate with Medical Directors on the development of interventions to support optimum health of participants including functional and quality of life, participant, and caregiver satisfaction through the efficient use of program resources and achievement of program goals
  • Develop a provider after hours strategy and approach for on-call arrangements to ensure 24- hour provider availability with consideration of participant and provider satisfaction
  • Provide consultation and support for Medical Directors regarding grievances, appeals, and IDT decision making processes
  • Actively monitor and digest current literature and best practices to make recommendations on changes to clinical pathways, policies, and processes as appropriate
  • Promote professional development of PACE primary care staff via regular group meetings, one on one sessions, and other strategies
  • Participate on the Quality Committee and other committees or workgroups as needed
  • Build a best-in-class culture of engaged care teams where doing what matters most to the patient drives care plans
  • Attract, coach, and develop clinical and non-clinical teams under the principles of our PACE model
  • Partner with the local and national operating leaders to implement the care model, prioritize how we build required capabilities, and evolve those capabilities over time
  • Oversee the work of IDT and other clinical leaders to ensure that evidence-based, high-quality, cost-effective care is provided to PACE program participants in accordance with the PACE program mission, current standards of medical practice, applicable regulatory requirements
  • Provide direct medical care to PACE participants, including the evaluation, diagnosis, documentation, and management of their medical conditions
  • For new markets and markets in staff transition, serve as the interim Medical Director including managing a patient panel on as needed basis
  • Serve as the main external clinical voice of the organization to major health system partners, medical network partners, community-based organizations, and corporate sponsors in the assigned market(s)
  • Lead relationships with community physicians and other providers to promote effective working relationships that reinforce our care model
  • Actively participate in community outreach, marketing, and activities that promote membership growth and referrals
  • Actively support the achievement of SCAN’s Vision and Goals.
