Requirements

  • A professionally certified pharmacy technician.

Responsibilities

  • Safely and aseptically prepare chemotherapy and other medications for patients.
  • Manage the Nucleus system and maintain accurate records of all transactions in the mixing room.
  • Mix chemotherapy and biologics within the biological safety cabinet (BSC) using proper aseptic technique.
  • Follow clean room guidelines and accurately prepare IV solutions and other sterile products.
  • Ensure all orders are processed and dispensed in a timely manner.
  • Check all orders for completeness of information, verify patient information, and proper physician authorization.
  • Interpret the appropriateness of a medication order before preparation.
  • Evaluate critical patient information prior to beginning admixture according to policy and procedure.
  • Check all orders for insurance approval before admixing.
  • Calculate cycle and day to ensure patients receive treatment on the correct day.
  • Review and check CBC results prior to pulling medications from Nucleus and notify RN of any concerns.
  • Recalculate drug dose as a verifier.
  • Calculate correct dosage volumes and select appropriate diluents.
  • Prepare chemotherapy in a syringe, label it, and place it beside the appropriate bag for RN validation.
  • Affix completed labels to bags or syringes as appropriate.
  • Initial the bag and distribute according to clinic guidelines.
  • Dispense medications following organization policies and procedures, placing them in a transport bag for nursing staff.
  • Prepare continuous infusion pumps, setting them according to physician orders, with RN verification.
  • Maintain CADD pump log, cleaning of the pump, and cleaning of the pump pack for next use.
  • Perform daily and monthly Nucleus duties as outlined in the Nucleus policy and procedure manual.
  • Inventory all medications at the end of each workday.
  • Utilize the Pharmacy Report to maintain appropriate 2-day levels of target drugs for the clinic.
  • Maintain the indigent patient drug program for the assigned clinic.
  • Properly document patient information in EMR.
  • Generate and review daily clinical reports and channel them to appropriate personnel.
  • Maintain all clinical logs.
  • Assist nursing staff with stocking supplies, clinic organization, and clean-up as needed.
  • Assist in providing orientation and training to newly hired pharmacy technicians.
  • Keep work area and equipment clean and neat.
  • Aseptically clean the BSC at the beginning and end of each shift, and in the event of a drug spill.
  • Wear appropriate personal protective equipment (PPE).
  • Properly dispose of all waste materials used when mixing chemo or non-chemo.
  • Clean BSC daily or as stated in policy and procedures.
  • Demonstrate knowledge of the appropriate use of PPE and chemotherapy spill kits in the event of a cytotoxic spill.
  • Follow all safety standards for handling chemotherapy and hazardous drugs.
  • Maintain current technician certification.
  • Provide documentation of attendance at least one seminar annually on aseptic preparation of parenteral products and on safe handling of cytotoxic drugs.
