As a Certified Alcohol and Drug Counselor, you will play a vital role in our mission to address the bio-psychosocial needs of our patients. This position involves performing social services functions and guiding patients in accessing treatment skills to achieve optimal emotional health. Your responsibilities will include offering basic consultation, delivering specialized clinical social work services, crisis intervention, and conducting short-term and long-term family therapy. We are seeking a dedicated individual who can provide information and referrals as needed, contributing to the overall well-being of our patients and their families. Requirements

  • Bachelor's Degree from an accredited college or university in social work or related field preferred; Master’s Degree preferred.
  • Experience and expertise in dual diagnosis, 12-step programs and substance abuse.
  • Must possess substance abuse license/certificate as dictated by the State of Georgia or equivalent, also Master’s License in Social Work or Counseling preferred.

Responsibilities

  • Interview patients to obtain substance abuse history.
  • Document findings in Dual Diagnosis portion of integrated assessment within 72 hours of admission.
  • Document updates when there are significant changes in patient condition or readmission in the last 30 days.
  • Communicate findings in progress notes in a clear, complete and timely manner.
  • Formulate a plan of care based on the interdisciplinary assessments within established hospital guidelines.
  • Ensure treatment plans include specific measurable goals/objectives/interventions and defining actions unique to each patient's needs and personal goals.
  • Update and revises plans as goals are met or patients' condition changes.
  • Ensure documentation in progress notes are based on the treatment plan.
  • Communicate on a regular and timely basis with relevant members of the treatment team.
