As our Chemical Dependency Counselor, you will empower patients to recognize and address at-risk substance use patterns, facilitating their journey toward improved well-being. Every day you will perform comprehensive clinical screenings using tools like SBIRT, AUDIT-C, and Plus 2, to identify risky substance use. You will be expected to assess individuals across the continuum of substance use disorders (mild, moderate, and severe) and facilitate appropriate interventions or referrals to treatment. To be successful in this role, you must possess strong empathy for diverse populations and a collaborative spirit for working within an interdisciplinary clinical team. Assures that the rights of the patient/family are respected and maintained. Allows for privacy and modesty in the provision of care. Identifies self by name and title to patient/family. Reports suspected cases of abuse/neglect, if identified. Assesses the physical, psycho-social, cultural, functional, high risk and needs of the patient and family Incorporates cultural and ethnic factors into care.
