About The Position

The Charge Nurse is responsible for assessing the patient’s physical and psychosocial condition on an ongoing basis throughout the continuum of care. This role involves developing and implementing a plan of patient care in collaboration with the healthcare team, evaluating patient outcomes, and completing required documentation accurately and in a timely manner. The Charge Nurse utilizes information systems to improve efficiency, demonstrates working knowledge of equipment and medications, and adheres to hospital policies and procedures. They act as a liaison for coordinating patient care activities, communicating the plan of care to the interdisciplinary team, patient, and family. This position also participates in performance improvement and risk management activities, provides education, demonstrates cost-effectiveness, and completes required continuous training. Additionally, the Charge Nurse complies with personal protective equipment requirements and attends meetings and in-service sessions. This role requires regular attendance and punctuality, and the performance of other assigned duties. In the Emergency Room, the Charge Nurse initiates assessments within 5 minutes of admission, collects key information, implements T-System documentation, performs ongoing assessments, and executes therapeutic nursing interventions ordered by the Provider. They determine the impact of interventions, document care based on standards, and manage care for patients with various conditions in the ER, interacting with patients and families with sensitivity and care, and maintaining professional relationships with the healthcare team. This position directly supervises licensed practical nurses, certified nurses’ aides, and ward clerks.

Requirements

Current Registered Nurse License in good standing with the State of Colorado.
BLS. ACLS, TNCC and PALS certifications required within six months of hire.
An ability to manage a sometimes-chaotic work environment related to changing patient needs, including working with patients with acute, chronic, and complex disease processes, and those who are dying.
Must possess the ability to function efficiently by working independently, in group settings and collaborating with various level of leadership.
Requires high level of integrity and detail oriented.
Thorough knowledge of principles, practices, and methods of Rehabilitation department operations.
Requires the use of discretion and judgment in both internal and external contacts, with ability to deal effectively with staff.
Must possess mature judgement and skill in dealing with employees and public while maintaining the highest degree of confidentiality with information obtained in the performance of duties.
Ability to follow all company policies, state, federal and stay within compliance of all SECHD requirements.

Requires strong people skills, verbal communication, and the ability to make immediate decisions based on sound theory.
Must be professional, flexible, caring, and compassionate and exhibit a congenial and sensitive attitude toward patients.
Ability to establish and maintain effective working relationships with patients, their families, and staff members.

Responsibilities

Assess the patient’s physical and psychosocial condition on an ongoing basis throughout the continuum of care.
Develop a plan of patient care reflecting significant nursing diagnosis and interventions to achieve desired patient outcomes, involving the patient/family and healthcare team in this process.
Implement the patient plan of care in collaboration with the healthcare team.
Evaluate patient outcomes, effectiveness of plan of care, and readiness for discharge.
Revise as needed and make referrals when indicated.
Complete required documentation in an accurate and timely manner.
Utilize manual and automated information systems to improve efficiency and effect productivity.
Demonstrate a current working knowledge of equipment, instrumentation, and medications.
Adhere to the standards, policies, procedures, and goals of the Hospital District.
Act as a liaison in coordinating patient care activities with nurses, physicians, and allied health professionals.
Communicate patient’s plan of care and significant changes to all members of the interdisciplinary team and to the patient, family, and significant others.
Participate in and support performance improvement and risk management activities.
Provide education as indicated after assessing the learning needs of the patient, family, staff members and students.
Demonstrate cost effectiveness by using supplies judiciously, anticipating needs for special supplies and equipment, and making recommendations for change.
Complete required continuous training and education, including department specific requirements.
Comply with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infectious body fluids, chemical disinfectants, radiation, asbestos, and other hazardous substances.
Attends meetings and in-service sessions, as directed.
Requires regular attendance and punctuality.
Performs other assigned duties.
Initiates assessment based on the patient's presenting problems or needs, the physician's medical diagnosis and within 5 minutes of admission.
Collects key information from patient, family, laboratory, and other medical reports. Implements T-System and documents assessment, including nursing diagnoses or other clinical judgments.
Completes ongoing assessments as determined by patient condition.
Performs therapeutic nursing interventions as ordered by the Provider consistent with primary and population-specific standards of care and practice.
Determines the impact of nursing interventions on the patient's condition and recognizes the patient's progress towards meeting desired outcomes.
Documents care based on standards of care and practice and/or clinical priorities for documentation including assessment and ongoing assessment findings, updated clinical priorities for documentation and therapeutic nursing interventions with special attention to patient education and discharge planning interventions.
Demonstrates ability to manage the care of patients with conditions in the Emergency Room.
Interacts with patients/families with a variety of developmental and sociocultural backgrounds.
Provides information to patients and families which reduces anxiety and conveys an attitude of acceptance, sensitivity, and caring.
Maintains professional relationships and conveys relevant information to other members of the health care team.
Relays information appropriately over telephone, pagers, and other communication devices.
