About The Position

Nazareth House is seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to serve as a Charge Nurse and provide clinical leadership to our caregiving team. This role is responsible for overseeing resident care, medication management, and staff support while ensuring compliance with all applicable regulations and community standards. The Charge Nurse partners with residents, families, physicians, and team members to promote exceptional, person-centered care and enhance the quality of life for every resident.

Requirements

  • Current and active California Licensed Vocational Nurse (LVN) license in good standing.
  • Minimum of two (2) years of nursing experience in assisted living, memory care, skilled nursing, or long-term care.
  • Experience with medication administration and resident care management.
  • Knowledge of state regulations governing assisted living and senior care communities.
  • Experience supporting residents with dementia and memory-related conditions.
  • Strong assessment, documentation, communication, and organizational skills.
  • Ability to respond effectively to emergencies and make sound clinical decisions.
  • Proficiency with electronic medical records and basic computer applications.
  • Valid Adult First Aid certification (or ability to obtain and maintain).
  • Ability to pass DOJ and FBI Live Scan background clearance.
  • Negative tuberculosis test within required timeframes.
  • Ability to complete required Relias training and ongoing education requirements

  • Previous experience supervising or leading caregiving staff preferred.

Responsibilities

  • Provide daily supervision and leadership to Med Techs, CNAs, RNs, and Caregivers.
  • Oversee medication administration and ensure compliance with state regulations and community policies.
  • Monitor residents’ physical, mental, and emotional well-being and promptly address changes in condition.
  • Respond to medical emergencies and coordinate appropriate interventions and follow-up care.
  • Maintain accurate resident records, service plans, physician orders, and clinical documentation.
  • Conduct resident assessments and participate in care planning.
  • Support resident admissions, including family meetings and room readiness.
  • Coordinate resident medical appointments and maintain communication with healthcare providers.
  • Build collaborative relationships with physicians, pharmacies, residents, and family members.
  • Assist with onboarding and ongoing training of care staff, including dementia care and medication management.
  • Participate in quality assurance activities and regulatory surveys.
  • Ensure adherence to infection prevention and control practices.
  • Report suspected abuse, neglect, or resident safety concerns in accordance with regulations and company policy.
  • Provide updates and recommendations to the Director of Clinical Services and leadership team.
  • Perform additional duties as assigned.
