About The Position

The Charge Nurse LPN will be responsible for ensuring appropriate medical follow-up, coordinating resident care with various healthcare providers, and completing assessments. This role involves managing daily and monthly staff scheduling for the clinical department, participating in after-hours on-call duties, and maintaining resident records. The Charge Nurse will also provide health promotion services, disease prevention counseling, and assist with medication administration and in-service programs. Maintaining a safe working environment and adhering to community safety programs are also key responsibilities.

Requirements

  • Current state licensure as Licensed Practical Nurse (LPN)
  • At least 1 year of experience in Assisted/Supportive Living
  • Exceptional communication and interpersonal skills
  • Must demonstrate the ability to serve with love, compassion, and dignity

Responsibilities

  • Ensures appropriate medical follow-up providing special instructions to resident and clinical staff related to resident care
  • Coordinates and reviews lab work, coordinating care with clinics, outside providers, and hospitals
  • Completes standardized assessment and assessment tools, may conduct initial assessment on prospective residents including offsite visits
  • Assists to complete and manage the daily or monthly staff scheduling for the clinical department
  • Participates in and has responsibility for after hour on call weekend duties to ensure resident care is provided 24/7/365
  • Performs assigned quarterly and annual reassessments on residents often utilizing EHR Also performs such assigned reassessments when the Resident’s health condition warrants.
  • Communicates with resident’s providers, other pertinent health care providers and family as to the ongoing health status of the residents.
  • Refers residents to appropriate medical care when needed. Conducts follow-up care as needed
  • Assigns the various personal care tasks contained in the resident’s service plans to the certified nursing assistants on an ongoing basis.
  • Assists, as assigned, in the community’s resident medication administration management.
  • Provides health promotion services, disease prevention counseling, and teaching self-care to residents on an assigned basis.
  • Provides, as assigned, general updates and progress notes to resident’s written records
  • Assists in the community’s in-service program
  • Assists in the community’s tuberculosis screening program
  • Maintain a safe working environment and always adhere to community safety program
  • Other general nursing and assessment duties, as assigned
