About The Position

Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois, founded in 1982 as Joliet Area Community Hospice. They provide exceptional serious illness care, hospice care, and grief support for adults and children. The majority of their direct patient care is delivered in the field, with team members providing care in homes, nursing facilities, or assisted living facilities. They also operate a 24-bed inpatient unit in Joliet, and non-clinical staff are based out of their Joliet administrative offices. The Chaplain role is a field position primarily covering Orland Park, Tinley Park, Lemont, and surrounding areas.

Requirements

Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree).
Clinical pastoral education certification required.
Minimum of one year’s experience in health care.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order.

Responsibilities

Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
Serves as liaison and support to community chaplains and spiritual counselors.
Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
Documents direct services and ongoing communication with community chaplains and spiritual counselors.
Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
