About The Position

Centric Home Health and Hospice is a private, local healthcare provider delivering Skilled Home Health and Hospice services. The Hospice Chaplain provides spiritual care and emotional support to hospice patients and families in home and facility settings. As a member of the interdisciplinary team, the Chaplain helps address spiritual needs during end-of-life care and supports continuity of care through collaboration with the hospice team.

Requirements

• Strong interpersonal and communication skills.
• Knowledge of community spiritual resources.
• Valid driver license, reliable transportation, and current auto insurance.

Nice To Haves

• Master of Divinity preferred.
• One year of chaplain or spiritual care experience preferred.

Responsibilities

• Assess patients’ and families’ spiritual needs and provide counseling and support.
• Participate in interdisciplinary team meetings and patient care planning.
• Coordinate visits from clergy or other spiritual advisors as requested.
• Maintain timely documentation and support quality improvement efforts.
• Promote continuity of care through collaboration with the hospice team.
