About The Position

As a Chaplain of Kindful Health you provide spiritual support, guidance, and resources to patients and their families. The Chaplain provides compassionate services to a diverse population including all faiths that are represented by patients, families, and team members, while at the same time, reflecting the mission and philosophy of Kindful.

Requirements

Graduate of accredited seminary or school of theology, or appropriate certification in pastoral ministry.
Minimum one (1) year experience as a healthcare pastor/chaplain.

Nice To Haves

Responsibilities

Adheres to all regulatory requirements and professional standards
Assesses patient/family to identify spiritual needs and develops a spiritual plan of care.
Collaborates with local clergy as needed.
Conducts services of prayer, worship and ritual for patients/families, as appropriate.
Provides individual patient/family counseling, crisis intervention, information and assistance with preparation of advanced directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
Work with IDT, participates in the ongoing POC by developing, documenting and evaluating responses to interventions in order to meet spiritual needs of the patient and family.
Provides consultation, education, and support to the interdisciplinary team on spiritual care and bereavement counseling.
Makes visits to patient/family prior to death and will be available to family during the bereavement period as needed.
Follows policies and procedures consistent with recognized standards of professional practice
Provide high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores
Collaborate with your Clinical Director and entire team to provide a "concierge" level hospice experience
