About The Position

To provide spiritual and emotional care to chronically and terminally ill patients using the Palliative Care Transdisciplinary Model. The chaplain gathers and evaluates data pertinent to the patient’s situation including psycho-social-spiritual/religious health in collaboration with the transdisciplinary team of doctors, nurses, social workers, pharmacists, patient, families, and care givers. The chaplain develops and implements a plan of care to promote patient well-being, continuity of care, quality of life and end of life goals. The chaplain models and collaborates within the organization and its disciplinary team to provide competent patient and family centered care respecting the cultural diversity of the population we serve. The chaplain facilitates the completion of advance directives (living will and power of attorney for health care). The chaplain provides information to patients and families regarding community resources.

Requirements

  • Palliative Care Transdisciplinary Model knowledge
  • Ability to collaborate with a transdisciplinary team (doctors, nurses, social workers, pharmacists, patient, families, and care givers).
  • Ability to develop and implement a plan of care.
  • Respect for cultural diversity.
  • Ability to facilitate the completion of advance directives.
  • Ability to provide information regarding community resources.
