About The Position

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children. The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices. The Pediatric Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Lightways Hospice and Serious Illness Care, either directly or through coordination of care with other spiritual counselors.

Requirements

  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree).
  • Clinical pastoral education certification required.
  • Minimum of one year’s experience in health care
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order.

Nice To Haves

  • palliative or hospice experience preferred.

Responsibilities

  • Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
  • Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  • Serves as liaison and support to community chaplains and spiritual counselors.
  • Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  • Documents direct services and ongoing communication with community chaplains and spiritual counselors.
  • Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
