The CFSP Hospital Liaison coordinates with hospital emergency departments (EDs) when youth are in the hospital without an identified discharge plan or placement. This role serves as the single point of contact for all communication related to youth awaiting placement, ensuring timely, consistent coordination between external hospital partners and internal care management (CM) and placement teams. The Hospital Liaison schedules and facilitates staffing meetings, tracks action items, and provides regular status updates to support safe, appropriate discharge planning. This field-based role in the Western Region of North Carolina enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. These roles are county field-based and require you to interact with patients, members, or providers in person four to five days per week.
