About The Position

Bost Inc. is seeking a full time Community Employment Supports (CES) Waiver Manager in Fort Smith, Arkansas. This role serves as the liaison between individuals and their assigned Provider Led Arkansas Shared Savings Entity (PASSE) to ensure the delivery of services aligns with the needs, goals, and objectives outlined in the Person Centered Service Plan (PCSP).

Requirements

• BS/BA degree or acceptable experience in Human Services or related field or a minimum of one year’s experience in Human Services or related field
• Valid driver’s license
• Reliable transportation with at least liability insurance

Responsibilities

• Implement PCSP plans for individuals on assigned caseload.
• Ensure budget information is completed and submitted timely.
• Ensure the individual attends all medical appointments and obtains required paperwork.
• Make monthly contact with each consumer on assigned caseload.
• Complete the home requirement checklist as needed.
• Ensure all staffing needs are met.
• Ensure the consumers plan of care is fully utilized.
• Other duties as assigned.
