About The Position

UPMC Community Medicine Incorporated is hiring a Full-Time Certified Registered Nurse Practitioner to help support UPMC Family Hospice. UPMC Family Hospice is a part of UPMC @ home, where we are committed to providing care through end of life in the home. As an interdisciplinary care team we would collaboratively to support the patient and family unit. The CRNP is a vital part of the care team and can help drive the plan of care, support the team with symptom management visits, help the providers determine eligibility, complete regulatory required virtual/in person F2F visits and provide coverage as the on call provider at the IPU and in the community hospice teams. Requirements

The successful completion of an approved nurse practitioner program is required
Current state RN license and CRNP certification are required
National certification, eligible for licensure by Pennsylvania board of nursing, eligible for certification as a CRNP by Pennsylvania state board of nursing
Current CPR certification is required
Act 34 Clearance required

Nice To Haves

Experience in primary care or internal medicine is ideal
Experience in Palliative or hospice is ideal
BSN, MSN is preferred
Professional nursing experience and/or nurse practitioner experience is preferred
